WAIVER & RELEASE OF LIABILITY
BY REGISTERING FOR ANY NEO YOUTH ELITE EVENT I HEREBY AUTHORIZE:
1. NEO Youth Elite to take photographs of my child named in this registration (“my child”) during club activities, and to display and otherwise use these photographs without charge solely for the purpose of promotional material in connection with NEO Youth Elite.
2. I also understand that injuries can arise by accident from the very nature of the NEO Youth Elite activities, and I hereby release and waive all rights to any claim or action against NEO Youth Elite, Ohio Bulldog Management, AND Garfield Heights High School its employees and volunteers, arising from injury, loss or damage to my child or to my child’s property except where such injury, loss or damage is caused by negligence.
3. I hereby authorize NEO Youth Elite, its employees and volunteers to seek emergency medical assistance and treatment for my child named in this application (“my child”) if the parents/guardians or emergency contact cannot be contacted.
4. I certify that the information and statements I input in registration both personal & medical are true and that my athlete is in good health and able to participate in the NEO Youth Elite Camps, Showcases, AAU Tournaments, and skills training.
5. I acknowledge that by entering the building or participating in the NEO event, all parties involved acknowledge and are aware of the risk(s) to exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “sever acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation variation thereof.
6. I agree to adhere to the designated spots on the bleachers for spectators and to maintain distance of 6 feet from other spectators not in my immediate family.
7. For myself and for the protection of others, I agree to wear masks at all times.
8. I agree that I will utilize sanitation stations that are throughout the building and use disinfectant wipes for basketballs that my child uses.
9. My child and I will refrain from post game handshakes.
